Daily home monitoring via blood pressure machine, weight scale, and pulse ox, with readings transmitted automatically and reviewed by a dedicated cardiac monitoring team every day. Medicare-reimbursable and deployable without adding clinical burden to your staff.
Chronic Care Management (CCM)
Structured care management for patients with two or more chronic conditions, including personalized care planning, monthly check-ins, medication support, and care coordination across providers. Medicare-reimbursable with established billing codes.
Principal Care Management (PCM)
Non-face-to-face care management for patients with a single high-risk complex chronic condition, including specialized care planning, medication management, and 24/7 care team access. Minimum 30 minutes per month. Medicare-reimbursable.
Cardiac Telemetry & Ambulatory Monitoring
Remote cardiac monitoring capabilities that extend your practice's diagnostic reach beyond the clinic, supporting telecardiology workflows and ambulatory cardiac monitoring programs for high-risk cardiovascular patients.

Practice & Patient Assessment
Our team reviews your eligible patient population and identifies which Medicare programs they qualify for. You get a clear picture of the revenue opportunity: how many patients qualify, what the reimbursement looks like, and what deployment would mean for your practice specifically.
Program Setup & Patient Enrollment
We configure the virtual care platform, handle equipment logistics, and manage patient enrollment. Your clinical team is trained on workflows and escalation protocols. Most practices are live within 30–60 days of agreement.
Go-Live & Ongoing Support
The dedicated monitoring team reviews patient data and manages proactive outreach when readings fall outside parameters. Your practice receives regular performance reporting with ongoing support to help you expand the program as your patient population grows.

RPM is appropriate for patients requiring daily monitoring of chronic conditions. Heart failure, hypertension, and complex arrhythmias are among the most common cardiovascular indications. CCM is for patients with two or more chronic conditions expected to persist for at least twelve months. PCM is for patients with a single high-risk complex chronic condition. Eligibility is reviewed during the practice assessment phase and we help identify which patients in your existing panel qualify.
Enrolled patients receive home monitoring equipment, typically a blood pressure machine, weight scale, and pulse ox. Readings transmit automatically to the monitoring team, who review them daily. Patients can also communicate with their care coordinator through a dedicated mobile app. The experience is designed to be simple for patients regardless of their comfort with technology.
Your physicians authorize patient enrollment and review escalated alerts when the monitoring team identifies a concern. Beyond that, the command center infrastructure handles daily monitoring, outreach, and care coordination. The goal is to extend your practice's care delivery without adding meaningful burden to your clinical staff.
RPM, CCM, and PCM are established Medicare programs with clear reimbursement codes. The specific billing structure and your practice's responsibilities are reviewed in detail during onboarding. We recommend confirming coverage and co-pay details with individual patients' insurance plans before enrollment.
Yes. There is no requirement to deploy all three programs simultaneously and no minimum enrollment threshold to get started.
